    Please arrange soft copies in the following order:

    1. Duly accomplished and properly signed application form (Download Form)
    2. Diploma or certificate of graduation
    3. Official transcript of records or certified true copy of grades
    4. If applicable: Thesis abstract and/or Special Problem including a fully signed acceptance page
    5. If applicable: Practicum certification indicating that the training is microbiology in nature or scope
    6. Proof of payment of the application fee.

    2. Previous exam result or the notification letter indicating that you qualified to take the exam
    3. If applicable: Other relevant document/s to support your application such as communication/s from CBd
    4. Proof of payment of the examination fee.

Effective February 11, 2019, all RMicro and SMicro professionals with expired certifications are required to comply with the established requirements, rules, and regulations on Continuing Professional Development (CPD).
We encourage all concerned professionals to update their credentials and fulfill the necessary CPD requirements to maintain their certification status.
