Applicant Requirements — Philippine Academy of Microbiology, Inc. (PAM)

Required application documents and fees for new applicants and for re-examinees, retakers, and re-applicants to the PAM RMicro/SMicro certification examination.

For New Applicants

New Applicant Requirements

Application documents must be merged into one PDF file, maximum 10 MB. Please arrange soft copies in the following order:

  1. Duly accomplished and properly signed application form (Download Form)
  2. Diploma or certificate of graduation
  3. Official transcript of records or certified true copy of grades
  4. If applicable: Thesis abstract and/or Special Problem including a fully signed acceptance page
  5. If applicable: Practicum certification indicating that the training is microbiology in nature or scope
  6. Proof of payment of the application fee.

  7. One (1) recent 2x2 ID photograph

For Re-Examinees / Retakers / Re-Applicants

Re-Examinee/Retaker Requirements

Application documents must be merged into one PDF file, maximum 10 MB. Please arrange soft copies in the following order:

  1. Duly accomplished and properly signed application form (Download Form)
  2. Previous exam result or the notification letter indicating that you qualified to take the exam
  3. If applicable: Other relevant document/s to support your application such as communication/s from CBd
  4. Proof of payment of the examination fee.
